Wes Side Sports Youth Program Waiver & Release of Liability

1. Agreement to Participate

I, the parent or legal guardian of the participant listed in this registration, give permission for my child to participate in Wes Side Sports programs, including but not limited to training sessions, leagues, camps, clinics, and events.

2. Assumption of Risk

​

I understand that participation in athletic activities involves inherent risks, including but not limited to injury, illness, falls, contact with other participants, and property damage.

I voluntarily assume all risks associated with my child’s participation in Wes Side Sports programs.

3. Release of Liability

​

I hereby release, waive, and discharge Wes Side Sports, its coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during participation.

4. Medical Authorization

​

In the event of an emergency, I authorize Wes Side Sports staff to obtain medical treatment for my child.

I understand that I am responsible for any and all medical expenses incurred.

5. Photo & Video Consent

​

I grant permission for Wes Side Sports to use photographs and video recordings of my child for promotional, marketing, and educational purposes, including social media, websites, and printed materials.

6. Code of Conduct

​

I understand that participants are expected to demonstrate respect, sportsmanship, and positive behavior.

Wes Side Sports reserves the right to remove any participant who does not follow program expectations, without refund.

7. Acknowledgment & Agreement

​

By registering, I acknowledge that I have read, understood, and agree to this Waiver & Release of Liability.

I understand that this agreement is binding and applies to all Wes Side Sports activities.
